Transconjunctival Under-Eye Fat Repositioning
Scarless internal transconjunctival repositioning of protruding under-eye fat pads to smooth tear-trough hollows and dark under-eye shadows.
Overview: Transconjunctival Under-Eye Fat Repositioning
Protruding orbital fat pads beneath the eyes often coexist with a depressed tear-trough ligament directly below them, casting dark shadows even in well-rested individuals. Rather than simply removing orbital fat—which can leave the eyes looking hollow over time—BONITA's Under-Eye Fat Repositioning (eyes.html) redistributes the bulging fat downward across the orbital rim to create a smooth, continuous lower-lid-to-cheek transition.
Surgical Technique & Candidacy
- Internal Transconjunctival Approach: Performed through the inner surface (conjunctiva) of the lower eyelid, leaving no external skin incision or scar.
- Septal Reset & Tear-Trough Release: The arcus marginalis and tear-trough ligament are gently released, and excess medial/central orbital fat is transposed and secured over the infraorbital rim.
- Ideal Candidates: Patients with prominent under-eye fat bulges and tear-trough grooves who have good lower eyelid skin elasticity and do not require external skin excision.
Recovery & Potential Risks
- Recovery Timeline: Because there is no external skin suture, most patients resume daily non-strenuous activities within 3–5 days as mild swelling and bruising resolve.
- Potential Risks & Side Effects: Temporary conjunctival chemosis (swelling), mild bruising, transient diplopia or foreign-body sensation, under-correction, or minor contour asymmetry.
